Obituary of Phyllis Simone

Phyllis Simone, 69, of Warsaw, formerly of Belleville, N.J.., died Wednesday at St. Luke Hospital West, Florence.
She was a retired actuary from NCCI Holdings Inc., Hoboken, N.J.
Survivors include her husband, Jim Simone Sr.; sons, Jim Simone Jr. of Walton and Ken Simone of Kingston, N.J.; daughters, Linda Simone of Kingston, N.J., Pam Simone of St. Roberts, Mo., and Angel Simone of Warsaw; five grandchildren; and six great-grandchildren.
No services are scheduled.
Visitation will be from 1 to 3 p.m. and from 7 to 9 p.m. Saturday at Chambers and Grubbs Funeral Home, Walton.
Mrs. Simone will be cremated.
Memorials are suggested to the American Cancer Society, 297 Buttermilk Pike, Fort Mitchell, Ky. 41017; or the American Diabetes Association, 644 Linn St., Suite 304, Cincinnati, Ohio 45203.
