Survivors include daughters Desiree, Alexis, and Jasmine Sams; sons Jordan, Elijiah, and Kalem Sams; father Jerry L. Sams Sr. of Cincinnati, OH; sister Tonya Sams of Dayton, KY; brothers Jerry L. Sams Jr. of Ft. Wayne, IN, Chris Sams of Dayton, KY, Joey Sams of Ft. Wayne, IN.
